Paperwork

We encourage all our patients to download our forms and take time to complete everything, with as much detail as possible. The information you provide, is an integral part of the service and treatment path we discuss during your appointment.

If you have difficulty accessing, understanding, or have any questions about completing a form, call our Danbury, CT office team – 203-743-4670.

Nice to meet you

Our office is easily accessible from anywhere in the Greater Danbury area – Get Directions.

In the waiting room, you will be greeted by a member of our office team who will review your new patient paperwork.

Examination and x-rays (oh my)

You will meet with your dentist to discuss your medical history and any oral health concerns you may have. Next, your dentist will likely examine your teeth, gums, and mouth, and take some ‘pictures’ of your teeth.

This initial comprehensive exam provides you and your dentist a status of your oral health, while learning how White Street Smiles can support your long-term oral health care. You may not receive a teeth cleaning during your first appointment: Each new patient relationship often starts with an initial exam so your dental team can understand your needs and concerns first, then tailor your hygiene treatment to you. This way, you’ll get the most out of your cleaning appointments and the time and attention you deserve.

No sales quotas, no hidden fees

Taking care of our patients has allowed us to thrive for over 40 years. We take pride in ‘telling it straight’, allowing you to make an informed decision. We’ll provide you a clear, detailed estimate for the cost of your treatment plan, in writing, so you know what to expect.

The beginning of a beautiful…

Relationships are built on trust and we’d like nothing more than to help you and your family maintain a healthy White Street Smile!
